Procedures and Guidelines for Self-service Measurement

Users must complete the required courses and instrument training. Only certified users are allowed to operate the instruments.

Charging Method

  • Academia : A 90% discount will be applied to the charges.(Price List)
  • Industry : User self-service is not available.

Application Process

Membership registration > Complete required training courses for each instrument > Complete occupational safety training > Complete instrument training and assessment > Access card application > User self-service

Required Training Courses for Each Instrument

Participate in and pass the required training courses for each target instrument before applying for instrument training and assessment.
(e.g., course 113C216-A is a prerequisite to the training and assessment of HF-102 67 GHz Measurement System)

Instrument Training and Assessment

To apply for the assessment: Go to the CS platform > Equipment Operation Training Application > Download the “Assessment form”

 
Access Card Application and Usage Instructions


  1. Access card application :
> Go to the CS platform
> Student Access Card Application
> Download the “Application Form” and check the information
> Submit the application form and deposit NT$300 to the customer service center to receive the access card.
 
  1. Usage instructions :
  • The intended users of the User Self-service are project users authorized by the project leader.
  • For those without a project, please submit an application through the “Project Family” portal.
  • Users with access cards may access the authorized areas.
  • Users who have passed the equipment assessment and received access cards, please proceed to the “Measurement Booking System” to make a reservation.
  1. Usage restrictions :
  • Users are not allowed to operate instruments or equipment without passing the corresponding assessment.
  • Access cards are prohibited from being lent to others.
  • Access cards are prohibited from being used to grant entry to multiple individuals. (Invited visitors, trainees, maintenance and installation personnel are not subject to this restriction)
  • Bringing unauthorized or unrelated individuals into the laboratory is strictly prohibited. (An application should be submitted in advance for an authorized visit.)
  • Individuals who have not passed the occupational safety training are not allowed to enter the laboratory.
  • A qualified user whose authorization have been canceled due to any reason must submit an application and pass the assessment again to regain the authorization.
  • Professors who wish to use the instruments and equipment should follow the same application procedure as other users to obtain authorization and apply for an access card.
  • If a user is unable to be present for self-service during the reserved time slot, please be sure to cancel the reservation online.
 

Access Card Holder Information Confirmation

 
  1. To maintain the accuracy of access card usage and member information, the center reconfirms access card holder information after the start of each semester (approximately in March and September). Please regularly follow the “News” section for more details.
  2. New students, including fresh and recent graduates, please resubmit the Basic User Information Form and electronic copies of the latest project reports or master’s theses to the Customer Service Center.
 

Access Card Return Policy

 
The process has been fully transitioned to online, and paper forms are no longer available. Please make sure that all information on the webpage is filled out. The account name and bank account number must belong to the same individual.
Once the information is filled out and submitted, the access card deposit will be directly transferred to the bank account in weeks.
